The "insert equation" icon on the rich text toolbar takes you to a dialog and doesn’t seem to be really usable...how to add an equation for instance?

To be able to use this editor, you need to install .NET Framework 3.5 for Equations editor that's supplied by Microsoft. 

Make sure you use Internet Explorer x32 on a x64 machine.

NOTE: ActiveX control is meant for IE only (as designed by Microsoft therefore Equations won't work in other browsers).
